1. Click on the “More” option located in the menu at the bottom of the screen.

2. Then, click on the “Access now” button to access SUS services;

3. To proceed, inform the access data;

4. As on the website, cardholder data must be displayed on the main screen;

For those who already have the traditional SUS Card, but do not know where it is or have lost it, the procedure is the same.

Just go to the nearest Health Post and ask for a duplicate.

Just enter your CPF number and tell the neighborhood where you live and the attendants will be able to print another one for you right away.
